janwas 657ec9aa45 WIP timer thread safety:
CAS functions now implemented directly in external asm files (avoids
wrapper functions)
reinstated cpu_CAS64 (checking ARCH_IA32 at every call site is onerous)
(temporarily disable timer monotonicity check)
