Added CStr and some other things to precompiled.h, to reduce build times.

Altered CStr/UniDoubler's macros, to avoid polluting/conflicting/etc.
Fixed headers in non-PCH builds.
Used the _d version of vorbisfile*.lib in Debug builds.
