5ee8354e99 Fix windows SEH hook when crashing in an std::thread
Follows 107d3d461f and other 'pthread->std::thread' diffs.

Windows uses Structured Exception Handling to allow reporting errors
(both C++ and hardware) nicely. This works by wrapping the code in a
__try __catch block.
The pthread wrapper did this automatically, but we now need to do it
explicitly for std::thread.

ba736916fc Clean up ThreadUtil, use standard C++11 constructs instead of custom ones.
ThreadUtil shipped a scope lock and a mutex implementation, which can be
removed since we now have these in the standard library.
This lets us clean up this header which get included everywhere (through
profiler includes).

4663ac0fe7 split debug_assert into ENSURE and ASSERT as discussed in a previous meeting.
the old debug_assert always ran and tested the expression, which slows
down release builds. wrapping them in #ifndef NDEBUG is clumsy. the new
ASSERT behaves like assert and ENSURE like the old debug_assert. Let's
change any time-critical but not-super-important ENSURE to ASSERT to
speed up release builds. (already done in bits.h and unique_range.h)

5228800b73 # Fixed Linux build
Use noncopyable instead of boost::noncopyable. (But maybe this should be
changed to the NONCOPYABLE macro instead?)
Use boost::filesystem::wpath::file_string instead of
external_file_string, since the latter varies between std::string on
Linux and std::wstring on Windows.
Use wcstombs instead of wcstombs_s.
Use rtl_AllocateAligned instead of _mm_malloc.

a34b759720 . split up lib.h/.cpp, include the remnants from PCH, remove (pretty much universal) include of it.
. timer, config: fix definition of ALLOW_RDTSC
. add movsx_be64 (for whirlpool), revise implementation, move to
byte_order, add test
. MAX -> std::max, remove those macros
. add timestamp to system_info as requested by philip

27b6ffe2d6 # housekeeping
sorry, update-workspaces + rebuild is necessary (moved boost/utility
into PCH)

- ps/ : committed additional documentation on behalf of Joe.
- lib/ : HAVE_C99 - replace with specific e.g. HAVE_NPRINTF; intended to
help with MacOSX compat (by no longer requiring us to lie about
STDC_VERSION)
- NO_COPY_CTOR -> boost::noncopyable

This was SVN commit r4825.

0bb0df5b2c # new year's cleanup (reduce dependencies, clean up headers)
- remove headers always included from PCH
- nommgr.h is only included ifdef REDEFINED_NEW (allows leaving out the
mmgr stuff)
- in lib/*.cpp, moved the corresponding include file to right behind the
PCH (catches headers that aren't compilable by themselves)
- byte_order no longer depends on SDL
- add debug_level (another means of filtering debug output; needed for
thesis)
- split posix stuff up into subdirs (lib/posix and sysdep/win/wposix).
makes including only some of the modules (e.g. sockets, time) much
easier.

90bfaf9c99 # housekeeping + bugfix in file trace recorder
entity.cpp: fix indenting (was using space instead of tab)
replace all instance of & operator=( with NO_COPY_CTOR
trace: fix bug reported by michael: if trace file is huge, indices would
keep on being recorded but data was cut off after a certain limit,
leading to invalid pointers (into garbage data)

a69ac0dee9 - fix w4 warnings
- add convenience macros for config_db(CFG_GET_SYS_VAL)
- VFSUtil::EnumDirEnts now uses flags instead of bool recursive
- UNUSED() for params, UNUSED2 (<- need better name) for variables
- config.h defines must be tested with #if (always defined) -> allows
detecting misspellings thanks to compiler warnings
- replace debug_assert(0) with debug_warn (its sole purpose)
- replace ScriptingHost::ValueToInt et al with ToPrimitive
- use nommgr.h to disable both mmgr and VC debug heap

This was SVN commit r2585.

468657479c philip points out precompiled.h -> string_s.h includes posix.h - too heavy for the PCH
replaced that with posix_types.h, which now defines size_t.
this change exposed a lot of code that relied on posix.h being included
implicitly; fixed those as well.
